employees working 16 hours or more per week. The amount of coverage is directly
related to employee’s earnings.

screens all newly hired employees for nicotine and nicotine metabolites at the time
of the pre-placement physical assessment to verify responses to the nicotine use
question in the pre-placement medical form. A smoking surcharge is assessed to the
benefits premiums for those benefit eligible employees who have tested positive
for nicotine and nicotine metabolites.We offer an opportunity to learn more about
and take part in smoking cessation programs.
